RELEASE AND ACKNOWLEDGEMENT OF RISK

I acknowledge that my child’s involvement and participation in the Activity involves certain risks and that injuries, death, property damage, or other harm could occur to me or others. I accept and voluntarily incur any and all risks of any injuries, damages, or harm which arise during or resulting from my participation and or my child's participation in the Activity, regardless of whether it was caused in whole or in part by the negligence of the Board of Directors of Bethel Baptist Church (“Iglesia Bautista Betel of Scarborough”), its affiliates, its officers, trustees, faculty, agents, representatives, volunteers, church member, church leaders, and employees by and through the Bethel Baptist Church (“Iglesia Bautista Betel of Scarborough”) (“Released Parties”).

I hereby waive all claims against any of the Released Parties for any injuries, damages, losses, or claims, whether known or unknown, which arise during or result from participating in the Activity, regardless of whether caused on whole or in part by the negligence or other fault of any of the Released Parties. I release and forever discharge the Released Parties from all such claims.

I agree to indemnify and hold free and harmless the Released Parties from all losses, liabilities, damages, costs, and/or expenses, including, but not limited to reasonable attorneys’ fees and other litigations costs and expenses, incurred by any of the Released Parties as a result of any claims or suits that I, or anyone claiming by, under or through me, may bring against any of the Released Parties to recover any losses, liabilities, damages, costs, and/or expenses which arise during or result from my participation in the Activity, regardless of whether caused in whole or in part by the negligence or other fault of any of the Released Parties.

I am 18 years of age or older and I’m the parent and legal guardian of the minor participating in the event.

I have carefully and thoroughly read and reviewed this Acknowledgment of Risk and Waiver of Liability. I understand it fully and voluntarily execute it.

Privacy

We are committed to maintaining the accuracy, confidentiality, and security of personal identifiable information ("Personal Information"). As part of this commitment, our privacy statement governs our actions as they relate to the collection, use and disclosure of Personal Information. Our statement is based upon the values set by the Canadian Standards Association's Model Code for the Protection of Personal Information and Canada's Personal Information Protection and Electronic Documents Act.

 

Medical information

Any Personal Information is collected for communication and administrative purposes. Additionally, any medical information is collected to provide care in the interest of health and safety for the minor participant especially in the event of injury or any medical emergency.

 

In the case of a medical emergency, the parent or guardian of the minor participant agrees and authorizes Bethel Baptist Church/Iglesia Bautista Betel, its agents and or representatives to transfer all medical information provided to the corresponding medical unit.

 

Any identifiable information, except for medical information during a medical emergency, is confidential and cannot be released without the express written consent of the parent or guardian.

 

Parents' and guardians’ responsibility

The purpose of this activity is to promote the religious advancement and development of the participants in an outdoor setting. Parents understand that although all precautions are taken, the participant's child may become sick, injured, or have a special situation that may present itself during the activity. Therefore, parents or guardians are responsible for picking them up at the campsite for any reason deemed necessary by the camp organizers.
